Seasons Hospice & Palliative Care of Wisconsin

We are a community-based organization on an ongoing mission to find creative solutions which add quality to life. We believe firmly in the patient/family focus of hospice care, recognizing that individuals and families are the experts in their own care.


Four Levels of Care:

General Inpatient

Short-Term

(average length of stay in hours to days)


Location

Nursing Home, Hospital, or Hospice Unit


Patient is having uncontrolled Symptoms (examples includes pain, respiratory distress, vomiting, bleeding)


Physician determines when patient is stable for transfer & hospice SW coordinates transfer to Home, ALF, or NH (Routine Hospice with family)

Continuous Care

Short-Term

(average length of stay is 24-48 hours)


Location

Assisted Living Facility, Nursing Home or Home


Physician determines that a nurse is required to stay with the patient for a few hours to frequently medicate and reassess the uncontrolled symptoms

Routine Care

Long Term


Location

Assisted Living Facility, Nursing Home or Home


Primary Caregiver: Family, Assisted Living or Nursing Home staff provide care and medications


Hospice Team visits and oversees the plan of care


Patient has terminal disease but has care plan in place for comfort

Respite Care

Max 5 days


Location

At contracted Nursing Home


NH staff care for patient while family has a "respite" period


Respite is scheduled by hospice SW


Programs Offered:

Open Access: Open Access is a hospice program that allows patients currently receiving medical treatments and/or experiencing intense psychosocial issues access to hospice services earlier; a blended model between curative and palliative care. For more info >

Namaste Care: Namaste Care uses person-centered approaches to improve quality of life through meaningful sensory activities that stimulate the senses, promote relaxation, and offer comfort and serenity to those living with dementia. For more info >

Music Therapy: The use of music enhances our lives as a means of teaching, celebrating and expressing ourselves and has been in place for thousands of years. Even when people are no longer conscious or speaking, we can console and comfort them with music. For more info >

Palliative Care: Our Palliative Care program provides clinical symptom management for people living with an advanced illness and emotional support for their families and caregivers. For more info >

Kangaroo Kids: Kangaroo Kids is a unique pediatric program dedicated to offering individualized and all-encompassing care to children and families who face a life-threatening illness with sensitivity and skill. For more info >


Inpatient Center:

A Seasons Hospice Inpatient Center provides a caring environment where patients can get help for their symptoms. It is a place where loved ones may spend hours or days with the patient, providing the warmth and comfort that family and friends can offer. For more info >

Medicare, Medicaid and the majority of private insurance plans have a hospice benefit that covers costs associated with hospice care.
